Your Visit

Getting Ready for Your Visit


If you have been told that you have or might have gynecologic cancer, or other gynecologic condition requiring surgery we are available to assist you. A consultation with Dr. Eisenkop may be arranged in two ways. Most commonly, your doctor will refer you to us. Alternatively, you may contact our office directly at (818)-905-1901 to schedule an appointment.

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

 

  • Your photo ID
  • A list of your current medications
  • Insurance information and card
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits, including CT scans, x-rays or biopsies. Please bring written reports from these tests with you or have them sent to our office in advance.
  • If you belong to an HMO/POS please tell us at the time you make your appointment.  Frequently an insurance plan requires an authorization which must be arranged through your primary care physician.
  • Download and complete a patient registration below.

Typically, your initial consultation will consist of a comprehensive history, review of your medical records and a thorough physical examination. After you have been examined, the physician will meet with you and whomever else you wish to be present. The results of your evaluation will be discussed and recommendations for further evaluation or treatment will be presented.

Family and Friends:  We highly recommend that you bring a support person with you to your appointment.  There is much information to absorb and a second pair of ears can help you keep it straight.  Having someone for support is especially helpful if you are feeling physically unwell or just emotionally overwhelmed by the circumstances.

Reception/Clerical

Carla is Dr. Eisenkop’s receptionist.  She will be greeting you at your visit. She is vital in helping you navigate the healthcare system. She will help you with appointments, authorizations and other paperwork associated with your care.

Clinical

Nicole is our clinical care coordinator.   Nicole will coordinate your labs, radiologic tests and post op or follow up visits.  If you have chemotherapy administered in the hospital she will coordinate appointments and necessary bloodwork.  Nicole is assisted by Maria.

Surgery Scheduling

If, after your evaluation by and consultation with Dr. Eisenkop, it has been decided that surgery is necessary you will meet with our surgery scheduling coordinator, Nicole.

Nicole will give you a date of surgery, hospital location orders for any pre-operative testing you will require.  These may include EKG, chest X-ray, blood and other laboratory testing.  You will be given a telephone number for pre-admission testing and  date prior to your surgery for pre-registration.

You will be informed of anything you must do prior to your surgery and the time you will need to arrive at the hospital the day of your surgery.  Nicole will give you an information packet that will contain any information you need to know regarding your hospital stay.

Nicole is also available by telephone to answer any questions you may have after you leave our office.

Chemotherapy

If it is determined that you will need chemotherapy, Dr. Eisenkop will supervise your regimen either in our chemotherapy suite or in the hospital as an outpatient or inpatient depending on your insurance requirements.  If administered in our chemotherapy suite the medications will be administered by a chemo certified oncology nurse.    Whether the chemotherapy is administered in the hospital or in the office,  Nicole will coordinate scheduling and labs.
